New Patients
As you are a new patient to our clinic it is important that you understand recommended dental procedures.
The Dental Hygienist you are seeing is a registered professional trained to help you to improve and maintain your oral heath as part of your overall general health.
It is also recommended that you see a dentist at least every twelve months for an examination and overall dental care plan.

Please complete the two forms below and bring along on your first visit. If you don't have a printer (don't panic) we can provide you a copy of the form in the clinic.
